Parent/Guardian Agreement
By signing this waiver, I, the undersigned parent or legal guardian, acknowledge and agree to the following terms and conditions in relation to my child’s participation in classes, performances, and activities at Music Stars Academy.

1. Assumption of Risk
   I understand that participating in performing arts activities, including but not limited to dance, acting, singing, acrobatics, and other physical and creative exercises, involves inherent risks of injury, illness, or accidents. I acknowledge that my child’s participation is voluntary and I assume all risks associated with their participation, including the risk of injury, illness, or accidents.
2. Medical Authorization
   In the event of an emergency where I cannot be reached, I authorize the staff of Music Stars Academy to seek medical attention for my child. I agree to assume full financial responsibility for any medical treatment deemed necessary for my child while they are participating in studio activities.
3. Release of Liability
   I hereby release, waive, and hold harmless Music Stars Academy, its owners, instructors, employees, and representatives from any and all claims, liabilities, or damages arising out of or connected with my child’s participation in any class, rehearsal, performance, or related activity, including but not limited to injury, illness, or accident.
4. Media Release
   I grant permission to Music Stars Academy to take and use photographs, video, or audio recordings of my child for promotional, advertising, and marketing purposes. These may be used in print materials, on social media, or on the studio’s website. I understand that no compensation will be provided for the use of these materials.
5. Code of Conduct
   I understand that Music Stars Academy expects all participants to behave respectfully and adhere to the studio’s rules and guidelines. Any inappropriate behavior, including bullying, disruption, or failure to follow instructions, may result in my child’s removal from classes or performances without a refund.
6. Payment and Refund Policy
   I acknowledge that all tuition fees and associated costs are non-refundable once paid, unless otherwise agreed in writing by Music Stars Academy. I understand that payment must be made in full before my child can participate in any class or activity. Late payments may result in my child being denied entry to classes or performances.
7. Acknowledgment of Understanding
   I have read and understand this waiver and release. I fully agree to its terms and voluntarily allow my child to participate in all activities at Music Stars Academy
